We provide Enhanced Family Care for individuals with Intellectual & Developmental Disabilities (I/DD) and Acquired Brain Disorder (ABD) across New Hampshire. This program is based on a “Shared Living” approach to provide quality care and support for individuals receiving services through the Bureau of Developmental Disabilities (BDS) and local Area Agencies. Shared Living programs are a housing option where people in the community open their doors to an individual with developmental disabilities and offer them a safe, loving place to call home. It is a very effective support system that gives individuals the opportunity to live a more interdependent life with a sense of belonging to their community.

Residential Placement Services

HopeWay provides welcoming, supportive homes where individuals can feel safe, valued, and truly at home. Our residential services offer personalized care in private, family-style settings with carefully matched and trained providers who take the time to understand each person’s needs, routines, and preferences. We focus on creating stable daily lives that encourage independence, meaningful activities, and community connection. Families can feel confident knowing their loved one receives consistent support, transportation to appointments and activities.

Community Participation Services

HopeWay supports individuals in staying active, connected, and included in their communities. Our programs encourage participation in cultural and recreational experiences—such as concerts, sporting events, and art programs—as well as volunteer and advocacy opportunities that allow individuals to give back. With personalized support and transportation assistance, we promote meaningful relationships, independence, and a fulfilling community life.

Nursing Services

HopeWay provides professional nursing services in accordance with New Hampshire regulations, ensuring high standards of health, safety, and quality care for individuals and peace of mind for families. Nursing oversight is integrated throughout the placement process, from initial assessment and provider matching through transition and ongoing support.

Our experienced Nurse Trainers oversee health-related needs, provide education and training to care providers, and ensure proper medication administration and healthcare coordination. We maintain ongoing oversight through regular quality assurance checks and continuous monitoring of health needs, while offering 24/7 on-call nursing support for urgent medical concerns. Through proactive involvement and responsive care, HopeWay ensures individuals receive safe, consistent, and well-coordinated nursing support in their home environments

Behavioral Support Services

HopeWay offers Behavioral Support Services designed to help individuals facing emotional, social, or behavioral challenges live safely and successfully in their home and community. Our services begin with comprehensive behavioral assessment and consultation to understand each person’s strengths, needs, and goals. Based on this assessment, we develop and implement individualized behavior support plans with positive, proactive strategies that promote skill development, reduce barriers to participation, and enhance quality of life.

Our team works collaboratively with individuals, families, and caregivers to ensure consistent support across home, community, and program settings. Services focus on building emotional regulation, communication, coping skills, and self-advocacy, while addressing behaviors in ways that support lasting well-being and community inclusion. Through ongoing training, monitoring, and adjustment of support, we help individuals thrive with dignity and confidence.

Day Program

HopeWay’s Day Program Services provide structured, engaging activities designed to help participants work toward personal goals while staying active in their communities. Each program is individualized with customized schedules, appropriate staff support, and opportunities for social connection, creative expression, and skill-building. Participants can explore job skills, volunteer, engage in arts and recreational activities, and develop independence, confidence, and meaningful community involvement.

Respite Services

HopeWay provides flexible, short-term Respite Services designed to support providers and caregivers while ensuring individuals continue to receive consistent, high-quality care in a safe, structured environment. Services are tailored to each person’s needs and include transition plans that mirror regular routines, helping maintain stability and minimize disruption. With temporary, reliable support, Respite Services give caregivers a much-needed break to rest and recharge, while supporting positive outcomes for both individuals and the teams who care for them.

Supported Employment

HopeWay helps individuals prepare for, obtain, and succeed in meaningful community-based jobs. Services include job readiness training, employment placement assistance, on-the-job coaching, and advocacy with workplace accommodations, providing the support needed for long-term success while assisting providers in delivering consistent, high-quality employment services.
